OK next question.
This form showed up today and I dont understand this section . This Authorization specifically includes the release of all information related to my physical and mental health and my insurance policies and claims, including, but not limited to, those containing diagnosis, treatments, prognosis, prescription drug information, alcohol or drug abuse or information regarding communicable or infectious conditions, such as HIV/AIDS. I dont see how it pertains to the accident and can I write in " As it relates to a loss occouring on or about 7/19/2008"? And "This authorization will expire 12 months from the date signed. " how about 90 days.? I am sure this is a standard form but I dont understand how being a crazy crack head 20 yrs ago with hepititis changes the fact that I was sitting at a red light and was hit from behind. I mean the information is protected for a reason. (no i am not really a crack head with hepititis, but I might be crazy)

You're right it is a standard practice for the insurer to order the release of all medical documents of the claimant in order to investigate into the claim process. You're required to give your consent to the insurer by signing the authorisation form.

What they need it for? the insurer will collect all your medical reports pertaining with the treatment for the injuries after the accident. However, I don't think that the claim process will get affected by the decease you had twenty years ago.

Yeah, it's standard and I could go into a long list of whys...but personally I've done the exact same thing you are talking about...I personally have changed the form to include the date of the accident thru present ONLY...then I went further and contacted my doc and made sure they caught that info on the form..the 12 months I left because, what was it gonna hurt, once it's settled they are not going to request additional information I assure you.

It is a standard form. My belief is that if you were sober at the time of the accident, it won't have any adverse impact on you whether or not you lead a S, D & rock and roll life in the past.
