Does my car insurance cover damage to my car caused by a loose tire rolling into my lane? I hit the tire after swerving to get out of its way.

If you have collision coverage, yes.

Hi tcope, can maintenance be an issue in this case? Can the insurer deny the case stating that the policy holder was negligent and hasn't maintain the car properly?

Coffee I think you misread the OP, a loose tire from another vehicle rolled into his lane....so the maintence of the insured vehicle would not come into play at all...
Now, if the insured's own tire flew off due to poor maintence, or if he had a blow out due to poor maintence his carrier would still pay but not for the tire, but all insuing damage....same for a mechancial failure fire, won't pay for what caused it (if it was a mech failure) but will pay for insuing damages.
OP i'm with tcope if you have collision coverage or UMPD it will be covered.

 yeah I did misread the OP but it seems that I've asked a question in return that might fit quite well into this context
Thanks lori for clarifying that the insurance company will not deny a claim blaming the maintenance issue. I was worried that they might do it since the owner was negligent with the maintenance.
