Received a form from union asking that we fill out the "subrogation acknowledgement". It looks like an emergency room visit. we have insurance so don't know why this form from union.

Is "union" the name of an insurance company or are you referring to a labor union? Depending on the reason for the visit to the emergency room, an insurance company may be entitled to subrogate the cost of the ER visit if it were the result of a third-party loss. Your medical insurance is intended only to cover first-party losses. If a labor union provides benefits under a "health and welfare" plan, then it, too, would be entitled to subrogation under the same theory.
