I've been a chain smoker once. But since 2007 I've completely quit it after getting help from my wife and a rehabilitation center. My question is would this fact that I was once a chain smoker affect my health insurance rates? I'm going to get one soon.

If you have not used tobacco products in any form for at least 12 months, you will not be classified as a tobacco user. The past is the past. Insurance is for the future.
