Can anyone tell how to claim disability insurance?

You can apply for the disability benefits in two ways:
• You send in your application online at www.socialsecurity.gov or,
• Call at the toll free number 1-800-772-1213 and request for an appointment with your local Social Security representatives. You can also set up an appointment over the telephone. In case you are a differently-abled person (like deaf or dumb), then you can give a call at the Social Security office in between 7 a.m to 7 p.m.

There is nothing in the OP's post to suggest that this has anything to do with Social Security Disability. In fact, it appears that there may be a disability policy involved, in which case the information about SSDI is completed off target.
