Calls to buy...or calls for a quote?
The best worksheets are provided by the carriers themselves.

You can definitely make the approach by asking them their personal details, such as, age, gender, name and address. Suggesting the customer the right policy is very important, therefore, you may also need to know about their requirements, financial status, what they are looking for in the policy and so forth. There is also a set of question that the agent may not ask, you need to keep those in mind as well.

I think that the question you're gonna ask to your probable client should be based upon the need analysis. Supposing, the client has called up for auto insurance, then your questions should be directed towards gaining information on his car, his driving record, claim history, whether he has an existing insurance or not, what amount he is paying currently for the coverage and like. The nature of the questionnaire will change with the type of the policy. I guess that make sense.
