How do I go about filing a claim against MetLife and the settlement reached involving The Death Master File class- action lawsuit?

You join the class to which you belong as part of the lawsuit. You'll have to find out who is representing the class and make them aware of you. If you search for it, you'll most likely find it.
A few years from now, you'll receive your $5.15.

You do not have to remain in the class. If you opt out of the class, you will have the right to bring your own separate action against the insurance company at your expense. Your attorney can petition for attorney's fees and costs as part of the damage claim.
