Try to get hold of a copy of your most recent bill and find out the company's contact information. Speak to the company's representatives regarding the cancellation of your policy and ask them to send a copy of their surrender form to you by mail or fax. However, do remember to ask them about any sort of penalty if you cancel your policy pre-maturely.

HOW CAN I CANCEL THIS POLICY
Contrary to the information above, if you are not the policy owner, you cannot do anything to cancel the policy except to claim that there is no insurable interest -- but since it involves your brother, even that won't work, because siblings generally are regarded as having an insurable interest in each other.
But, since you seem to enjoy signing blank documents, I have a blank check with your account number on it. How can I get it to you to sign it for me?

Max,
I would think that he would have a reasonable chance of getting the policy rescinded if he let them know that no questions were asked of him and that he signed a blank form.
Since the form was blank, how do you know that he isn't the owner?
The bigger problem is that he has no idea which insurance company is involved. At least, he isn't paying for the coverage.

I guess he needs to look over his shoulder from now on. His brother may be out to get him for the life insurance money.
I suppose it wouldn't be too farfetched for him to ask his brother about the policy.
