Yeah, and may be some other personal information, such as- date of birth etc. the insurance company can ask about the information that they may have in their database. Some may simply write the check on your name once you submit all the relevant documents.

The insurance company will most likely need your social security number as there are too many people in the world with the same name. Every Insurance company is different. Gary and LifeIsGood are correct. They will want to establish relationship as well as name.
