Is there a form for the benefits cost recovery depreciation for a total loss of a home?

Form? It's unclear what you are asking about. Recovery depreciation is a number. There could be a "form" that gives a break down of the Actual Cash Value amount and the Replacement amount of a loss.

Not quite sure what the question is here. If it is a total loss and a replacement cost policy they may very well just pay out the claim upfront. There are however several different forms used during the claims process, if the insurance company has requested you to fill out a form, I would recommend asking them to send you a blank copy of the form they are looking for.
