I am not personally in the underwriting business but from I have been told, the main factors are going to location, valuation and background of the insured.
You are not required to have homeowners insurance simply because you own a home however, if you have a mortgage they will most likely require you to insure the property in order to protect their investment.
That being said, to go without insurance can be very risky.

Even if it's not required, home insurance is extremely useful for many reasons. Homeowners insurance can extend from property insurance on the outside to contents insurance on the inside. It also may cover liability as well, just in case someone is injured at your home.
But there are also some downsides to home insurance. Find more info at Insurance Center Associates.
