How rates are determined

At present I am 28 and my husband's age is 35 and my kid is of 4. We are all having good health and non-smoker.Now we have decided to buy a health insurance policy for our family. Could anybody have the knowledge that how the rates are determined?

I have not so much of knowledge regarding the health insurance. But I think the premium rate will be based upon the elder most member of the family. For your case the rate will be based on your husband.
