Get pay slips establishing that you have had premium deductions made, then contact the insurer to find out whether they have recieved premium payments from your employer.
If they have, then it may just be a reporting error, but if they haven't your emplyer may be taking advantage of you - deducting wages and not passing them on to the employer. I'm not sure how US law treats this but I'd assume that you would have a right to sue them for not only your deducted amounts, but also for any medical costs and possibly punitive damages.
Does your employment contract state that they have a medical insurance scheme? If so even better.
Bottom line: If your employer has been deducting money from your wages for medical insurance, and then not paying that on to the insurer you may need to take legal action against your employer. Fraud springs to mind, but there could be more fitting charges.
What you want is proof that they have been deducting your money. From there you need to find out whether the insurer recieved it or not.
Is suspect you might need legal advice.
