I believe your questions is about a loss settlement check? If there is a lein on your vehicle and the insurance company knows about it they will make the settlement check payable to your leinholder and you. That way you can pay off the loan on your vehicle or the leinholders interest in your vheicle.

If a first party claim, (your carrier) they have a contractual obligation to protect your lein holder...If third party and NOT a total loss the responsible carrier can pay you direct.
