What form is required to disenroll from Texas Health Spring HMO system?

Health Spring is a Medicare HMO, if I'm not mistaken. You have to follow the Medicare rules on disenrolling in one plan and enrolling in another.
You have probably waited too long to do this for 2010. Your next opportunity will occur between Nov 15, 2010 and Dec 31, 2010, and become effective Jan 1, 2011.
There is no specific form to disenroll, but it happens when you enroll in another plan.
Are you dissatified with Health Spring for any reason in particular?

Please tell me how I can disenroll from your health plan and go back to original medicare. We joined friday the 31st so we are not sure that we are in your system yet.
This is very timely, so we must move back immediately.

This is very timely, so we must move back immediately.
You have waited too long to make this effective prior to July 1, 2011. You can file your application to return to Original Medicare anytime between now and May 15, 2011 (I don't know why you would really want to do that), and your Medicare Parts A & B will be restored on July 1.
In most areas, where available, Medicare Advantage plans represent a significant added value over "Original Medicare". In many areas (such as here in Southern California), there is no additional premium for the plans.
