Very sorry for your loss.
It sounds like she may have been covered by a term life policy, and if that's the case, there may be no coverage after age 85. If your mother had coverage as a city employee, it would most likely have been under a group policy.
The Medical Information Bureau has a "policy locator service" ( http://www.mibsolutions.com/lost-life-insurance/ ) , but, as far as I know, it does not include group life insurance, so you may not want to spend $75 to find that out.
If she was a member of a labor union, the union may be able to provide additional information, as might any of her former co-workers you might be able to locate.

She paid, and they covered her. If she had died while the policy was in force, the company would have cut a benefit check to the named beneficiary. If there wasn't one, it would have been sent to her estate.
The error here is not having properly planned. Sorry for your loss (potentially in more ways than one). If the employer tells you that there is no policy, I'm not so sure I'd push the issue as there likely no reason for them to want to not see you receive the check.

amount paid by your mother to insurance company can be traced if you see the checking account transaction history (if you have printed statements) and from that you could be able to trace the insurance company.
Once you get to know the insurance company's name,I don't think its very difficult to get trace out the policy.

And if the policy was employer-sponsored group life insurance paid for by the employer, perhaps with payroll contributions from the employee, now what do you do, DIMG786?
Please don't give out more bad information.
