My mother-in-law wants to travel to the USA for a vacation with family. She is 91 years old, quite fit. She had a mild stroke several years ago, otherwise she is in good shape. Can you advise on insurance to cover medical requirements during her visit?

Not going to happen. To elderly and not a legal resident. Unless she is planning to take up residence here. And then, it's probably not going to happen.
The good news is that the Medicaid program will pay hospitals and doctors expenses if needed. Almost no questions asked.
