SNFs Push Patients Out!

I'm a social worker for a large Ohio hospital. I am constantly inundated with families coming to me for help because their loved ones were d/ced from nursing homes and unable to care for them at home. What are your opinions about how/why this is happening? Where is the system failing us? Is the parameters of Medicare? Naive families?
