a woman pulling out of a grocery store into traffic pulled out in front of my son then stopped and he hit the back of her car is he at fault

Usually. She had enough time to pull away from a stopped position and get completely in front of your son. You son should have seen her the entire time she was pulling out and slowed immediately in order to avoid the collision. There is some room for negligence against the lady as the argument is that she did not yield the right away and pulled out right in front of your son. Most likely there is some negligence on each party. Your state laws then determine how it will be handled.
