If a pedestrian is hit by a car in a no-fault state, whose insurance is billed, the pedestrian's or the driver's?

Depends on the state. Most that I know of go to the pedestrian's own auto policy if they have one. Some then go to a resident's auto carrier but most go to the other drivers policy if the pedestrian does not have their own coverage.

In a no-fault state, you'll have to use your insurance. If you don't have an insurance then consider the car's insurance that injured you.
