Is standard restraining order filed

Is standard restraining order filed by ex spouse in 2006 at legal separation still valid after dissolution was filed by husband almost a year later and judgement for divorce was granted with only real property issues pending (1 home). husband remarries in 2008 changed beneficiary to current spouse in 2008. Husband who remarried dies years later insurer pays current spouse.
after receiving a dispute phone call from ex wife insurer stops payment on check after cleared current spouse bank ex spouse sent no proof or document to insurer until later. 72 days later she the insurer an SRO filed at legal separation in 2006 ex-spouse never amended SRO at time dissoloution was filed and no proof of service showing the SRO was amended at dissolution. There was no judgement ever entered regarding insurance beneficiary in divorcee decree nor is their any mention of life insurance in divorce decree (this is not a company policy) but through a large insurer. Insurance benefit paid to current wife insurer placed a stop payment after funds were paid to current spouse and cleared her bank now current wife account overdrawn by $$$, insurance company still holding funds saying they don't know who to pay. Is Standard Restraining Order filed at legal separation valid after dissolution was filed? and divorce was granted?
