Can an insurance company deny you as the beneficiary if they scanned an illegible copy of the designated beneficiary statement? Can they deny you as the assignee if you have a notarized copy but they claim they never received it?

Notarized, schmotarized. All of that is 100% meaningless. The insurance policy invariably states that the insurer must receive any notice of assignment during the lifetime of the insured and that an assignment will not be effective under any other circumstance.
However, having the notarized form might be good enough to press a claim against the beneficiary after he/she receives the insurance proceeds.
