In a nutshell, the screening process by which individuals are rated by medical insurance companies is called medical underwriting. It basically relates to the risk involved in insuring a person. It is illegal in some states too (like Washington). It would be better if you could put down the exact thing that you want to know about it.

I guess you want to know about the process of medical underwriting. Well, here is a brief guideline for you- Medical underwriting is related to health insurance the same way underwriting is related to the other forms of insurances. The medical underwriters peep into your medical history to determine the level of coverage and premium rate. They normally consider
1. The list of your ailments
2. How many times you have been hospitalized in the past?
3. Are you undergoing any treatment currently?
4. Have you been subjected to any medication or therapy in the recent past or not?
5. prescription drugs
Depending upon this above information they will decide upon your premium, inclusion of riders (optional) and pre-existing conditions exclusion period.
