Depends on what type of insurance we are talking about. But in any case I don't think this is actually what is being done. I think what they are doing is stating that the treatment is not "reasonable". Even then, it's a fine line for the insurance company to walk.

More detail is required. In general the insurance company can base decisions on what is reasonable - they will rely on Doctors advice to some degree.
If you want to explain a bit more I am sure you'll get better answers.
