- Date on which you would like the insurance to end
- Reason why you want to cancel your policy (might not be needed in many cases)
- Effective policy dates (although the insurance company will have it, but it would still be a good idea to include)

Well, I'm not sure if we have to submit our cancellation letter with the agency. But I guess, the letter should be headed to our insurer. Ultimately, it has to reach the carrier who has the authority to issue or cancel policies.
