- Your mobile or any other telephone number
- Your insurer's name and address
- Date on which you would like the insurance to end
- Reason why you want to cancel your policy (might not be needed in many cases)
- Effective policy dates (although the insurance company will have it, but it would still be a good idea to include)

I have decided not to go through my insurance and pay out of pocket for small car accident to total car dammages of maybe around $3,000.00 I was at fault.
However, I need to write a letter to the other driver stating that I be released after settling with him the car expenses, so that he does not come back at me along with the 2 passengers withing the next 2 years saying that they are suuing me for whiplash, and this I see could be possible from them. Please show me a sample of such a letter that I could present to them at time of settling.

It would be a bigger mistake to provide you with such a letter than to turn this claim over to your insurance company to handle. That's what you pay premiums for.
To think that your premium will be unaffected because you pay a claim privately is foolhardy. An accident with this level of physical damage is reportable to the DMV and will appear on your driving record. It is your driving record, not so much your claims experience, that determines the bulk of your premiums.
Signing a waiver for payment of physical damages does nothing for you when it comes to any personal injuries and related expenses. In fact, making a payment for physical damages will be seen as PROOF OF RESPONSIBILITY for personal injuries.
You can get the other driver to sign any letter you want, perhaps even waiving his right to personal injury loss, but that person's signature cannot waive the rights of the passengers to their damages, if any. You will still be responsible for those.
If you do not turn the claim over to the insurance company at this time, they are not obligated to provide coverage or a defense for you later. Do this right, and turn the claim over to your insurance company and let them handle it. You are in way over your head on this one. Letters like you are thinking of will only sink you deeper.

I have couple questions. I currently have auto insurance with Farmers Insurance Group which I want to cancel. On my insurance documents they have Company name as MID-CENTURY INSURANCE COMPANY (a stock insurance company) although it's with Farmers insurance, what company name do I put on my cancellation letter? My insurance agent is based out of Woodland Hills, CA although they have an issuing office address in Hillsboro, OR on my insurance doucments. Which address do I put down? and last question... in the part "FirstName Lastname or Cancellation Department" who's name are you reffering to, my agent's name?

Write to the company whose name and address is on your insurance documents. Mid-Century is a Farmers Company.
BUT . . . DO NOT CANCEL before you have replacement coverage in effect. If you cancel and cannot get coverage through another company, you may not be able to reinstate your former coverage. There is a right way and a wrong way to do this, and cancelling existing coverage before you obtain new coverage is 100% the wrong way.
