How does health insurance work in terms of payment?

I take it you are wondering about whether you have to pay the money up front and get reimbursed or whether the insurer pays the hospital directly?

in almost all cases I'd expect the insurer to settle directly with the hospital.

Some people would refer to it as a 'cashless settlement'. It is done if the health care unit where the insured got treated falls within the carrier's network. Otherwise, the insured needs to bear the expenses initially and gets reimbursed for the medical bills later.

pretty tough to do if the treatment ranges into the 10k+ mark, are there legislative provisions in place that require insurers to pay out upfront if the insured can't fund the treatment at the front end?
