What is a declarations page?

My agent sent me a letter and asked me for a copy of my prior declarations page. What is a declarations page?

A Declarations Page is the part of a policy that lists your specific information. For example, a homeowners declaration page will list your name, the address of your home, your policy number, the effective and expiration date of your policy, the premium for the coverage you have purchased and the limits of each of those coverages.

its helpful for an agent to have this information in reviewing your coverage...comparing apples to apples. also when changing from one company to another, most companies offier a nice discount for a solid insurance history and showing no gaps in coverage when transfering to them...a dec page can be used to document qualifiying for that discount.
