can you use the health insurance in other country?example if I have in health insurance in Philippines,can I use it in U.S.A.?

You may have to ask the agent or can go through the policy document to know exactly what's covered. At times, depending upon the business strategies and financial capacity the insurer may extend the coverage beyond the geographical boundary. However, if you are contemplating the idea of permanent residenceship abroad you may have to obtain a health policy there. Your present health plan may only continue coverage for a certain period of time.

I don't know about Phillipino coverage, but I can tell you that most PPOs in the States allow you to leave the country and still be covered. They may require you to pay for services in advance and then submit to them for reimbursement, however.

But, how would it work then? Will the insurer ask for more contribution from you? or, you would be responsible for the co-pay only?

I can only speak fluently about Blue Cross, because I worked for them, but your benefits would be the same. If you have a copay for office visits, then you would just owe the copay once the claim was paid. This would be for medically necessary services only, though. I wouldn't get a routine physical or that colonoscopy while your in Brazil. ;)

My bet would be that you would be covered at the regular cost sharing percentages if you where being treated for an emergency medical procedure but if you just went for something routine like a physical than you might have increased cost sharing percentages similar to if u where out of network in the states. It really is going to depend on your insurance company though, I would just call an ask.
